Professionals committed to improving children’s outcomes through a consistent transition approach meet on a regular basis to review and continue developing the ‘Birmingham Transition Approach’.
Developed with children's outcomes at the centre of the process, the Transition Approach also meets the needs of children's families and the professionals working with them.
